Wellness has been an increasingly important aspect of health care in recent years. As a Trust participant, you have access to a vast array of Health and Wellness information and initiatives that are offered by the Trust’s medical claims administrator, Highmark Blue Cross Blue Shield.
The Health and Wellness section of Highmark’s website provides valuable information on condition and treatment topics and allows participants to search the Symptom Tracker, access the Medical Encyclopedia and search Healthy Living Topics. The site also offers personalized, web-based lifestyle improvement programs that participants can participate in at their convenience.

Living a healthy life means more than regular doctor visits—it’s helping you find time for the things that matter most. Trust participants have access to a wellness discount program developed by Highmark. The program features discounts on a variety of health-related products and services.

Preventive care plays a key role in identifying potentially serious health risks at an earlier, more easily treatable stage. The Trust health care plan includes 100% coverage for preventive care services. This means that there are no co-pays for these services.
Participants are encouraged to have an annual physical exam and regular preventive health screenings. Preventive health screenings provide an early warning system. Ask your doctor about screenings for cholesterol, blood pressure, and glucose.
During your annual physical exam, it is recommended that you provide your medical provider with a copy of Highmark’s Preventive Schedule so that the provider can easily determine what Preventive Screenings are appropriate and necessary.

Blues On Call is Highmark’s comprehensive health information and support program that provides you with up-to-date, easy to understand information about medical conditions and treatment options. A registered nurse Health Coach is available 24 hours a day, seven days a week to help you make informed health care decisions, maximize your ability to take care of yourself, and follow your prescribed treatment plans to improve your health. Blues On Call offers three levels of health coaching and support:

Care Management is a voluntary program in which a care manager, with input from you and your health care providers, assists when you are facing and/or recovering from a hospital admission, suffering with a chronic medical condition, dealing with multiple medical problems or facing catastrophic needs. Highmark care managers can provide educational support, assist in coordinating needed health care services, put you in touch with community resources, assist in addressing obstacles to your recovery such as benefit and caregiver issues and answer your questions.
Highmark care managers are a free resource to all Trust participants. If you had a recent inpatient hospital admission or have a chronic medical condition, you may be contacted as part of the Outreach program. If your claims history indicates that your needs appear to be more complex, you may be contacted by a care manager from the Complex program. Even if you are not contacted, you are always free to call and request care management if you feel you need it by contacting Member Services at the telephone number listed on the back of your ID card.
